Please note if you will be:
Under 18 years of age by June 16 - Fill out YOUTH APPLICATION, MEDICAL FORM, AND CODE OF BEHAVIOR
Between the ages of 18 and 20 by June 16 - Fill out YOUNG ADULT APPLICATION, MEDICAL FORM, AND CODE OF BEHAVIOR
Over the age of 21 by June 16 - Fill out ADULT APPLICATION, MEDICAL FORM, AND CODE OF BEHAVIOR
All medical forms must include a copy of the insurance card AND be notarized. We will have a notary present at the meeting.
All paperwork is due by April 1.

INTERESTED IN MISSIONS? Any current 8th graders through college can join us for Catholic HEART Workcamp Philadelphia 2012. Our trip will be June 9-16, 2012.
To register, please download the info packet, complete the Letter of Commitment and return to parish office with a $75 deposit.
