Who provided your baptism formation/preparation?
REQUIRED
The world-wide Catholic Church has set a requirement that all parents of children receiving baptism receive some instructions on the sacrament. Inasmuch as the sacrament of baptism creates a permanent mark on the soul of your child, we want to ensure that you are "on-board" with all that baptism brings to your child, and thus to your family.

Requirements for being a godparent at Baptism.
The godparent is to be named by the parents of the infant or by the adult to be baptized.
The person should be over sixteen and have received
ALL
three Sacraments of Initiation
(Baptism, Eucharist, Confirmation)
and be leading a life of faith in harmony with their undertaking of this role.
At least one godparent is required, and if two, then there should be a godfather and a godmother.
A baptized person belonging to a non-Catholic ecclesial community may be a withness to the Baptism along with a Caholic person as godparent.
With respect to Godparents, what do you desire?
REQUIRED
There are certain circumstances (for example, an emergency baptism when the child may soon be dead) where Godparents are NOT required for baptism. We do want to have at least one Godparent for scheduled baptisms at All Saints. If you are finding it difficult to find a Godparent, we can help you. Please let us understand the situation. If you desire more than one Godparent, you may have a second Godparent, provided that they are of the opposite sex (i.e. one Godfather, one Godmother).
One Godparent (Godfather)
One Godparent (Godmother)
